Purpose of Agreement:

This agreement sets forth the arrangements concerning guardianship, custody, and visitation rights of the child, aimed at ensuring the child’s well-being.

Custody and Visitation:

The parents agree to share joint legal custody of the child. The custodial schedule shall be as follows: Mother shall have primary physical custody, with visitation rights granted to Father every other weekend and on alternating holidays.

Child Support:

Child support payments shall be made by the non-custodial parent in accordance with the Illinois Child Support Guidelines, with payments due on the first of each month.
